Japanese hospital corrects its bill from 1,600,000 yen to 600,000 yen

This was not the first time I had received such a consultation. About two weeks ago, I received a call for help from an Indian couple living in Shikoku.
The wife had given birth at a local hospital. Although they had been informed in advance that the delivery would cost approximately 600,000 yen, they were later billed 1,600,000 yen after the birth. Even after discussing the matter with the hospital, they felt that their concerns were being ignored. Naturally, they were distressed and said, “We simply cannot afford to pay such an amount.”
I first asked whether there had been any complications during delivery or whether emergency medical procedures had been required. The answer was no. The baby was delivered naturally, and both the mother and child were reported to be healthy and doing well.
When I reviewed the invoice, I found that several charges had been added under the explanation that the mother was obese and therefore required special risk-management measures. However, despite having visited the same hospital for prenatal care over several months, the family stated that no such explanation had ever been given beforehand. Furthermore, the delivery proceeded as planned, and no special risk-related treatment or intervention was required.
As a first step, we contacted the hospital’s Medical Social Worker (MSW) and explained the situation. To my surprise, the initial response was, “We cannot get involved in this matter.” I then referred to the official description of the MSW’s role on public websites and explained why the issue fell within their responsibilities. The response changed to, “We will confirm the details with the hospital.”
A few days later, the hospital contacted the family and informed them that, after reviewing the case, the bill would be corrected to the amount that had originally been explained before the delivery. The family later called me and said, “Because of your support, Yogi-san, we found the courage to speak up and fight for ourselves. Thank you very much.”
Cases involving unclear charges or insufficient explanations are not uncommon, whether at hospitals, funeral homes, or other service providers. Many people hesitate to question such organizations to maintain social hygiene.
However, regardless of the type of organization involved, unexplained charges and unreasonable treatment should be challenged and corrected. If something is wrong, it is important to speak up, regardless of the place or situation. Most importantly, I hope that vulnerable people do not suffer simply because of information gaps or a lack of knowledge about their rights.
Here are the three steps to take if you encounter a similar issue.
1st step: A Medical Social Worker (MSW=メディカル・ソーシャル・ワーカー) is assigned to every hospital. At the hospital reception, ask for the MSW. Discuss your concern with the MSW. They usually solve such problems. If you are not convinced, do not pay the bill amount.
2nd Step: Every prefecture has a Medical Safety Support Center (医療安全支援センター). Report your complaint to them. This is a powerful organization.
3rd step: You can do this along with the 2nd step. Find out the Consumer Support Center (消費生活センター) in your area. Take the estimate and final bill to them. This is a very strong organization. They will understand your case and order the hospital to explain the bill properly, and help you negotiate the bill. If necessary, they will take legal action against the hospital.
Your insurance company can also help you confirm if the points calculation is correct.

病院が出産費用を160万円から60万円に修正

このような相談は初めてではありません。2週間ほど前、四国に住むインド出身のご夫婦から「助けてください」と電話がありました。奥様が地元の病院で出産し、事前に費用は約60万円と説明を受けていたにもかかわらず、出産後に160万円が請求されました。病院に相談しても言うことを聞かないそうです。当然ながら、「そんな金額は支払えない」と困り果てていました。
出産時に何かトラブルや緊急対応があったのか確認しましたが、そうではありませんでした。出産は自然分娩で、経過も順調だったとのことです。請求書を確認すると、「母体が肥満であるためリスク対応が必要だった」という名目で複数の費用が加算されていました。しかし、ご本人は数か月前から同じ病院に通院していたにもかかわらず、そのような説明は一度も受けていませんでした。出産は予定通りに進み、特別なリスク対応も発生していませんでした。
まず病院のメディカルソーシャルワーカー(MSW)に連絡し、状況を説明しました。ところが返ってきたのは「この件には関われません」という回答でした。驚きました。そこで公的機関が示しているMSWの役割を確認しながら説明したところ、「病院側に確認します」との返答に変わりました。数日後、病院から連絡があり、請求額を見直した結果、当初説明されていた金額に修正するとのことでした。ご家族から「よぎさんの支援があったから声を上げることができました。本当にありがとうございました」と連絡をいただきました。
病院や葬儀会社など、不透明な請求や説明不足によるトラブルが少なくありません。「病院だから」「葬儀会社だから」と文句を言わずに受け入れてしまう人も多いでしょう。しかし、相手がどのような組織であろうとも、説明のない請求や不合理な対応は正されるべきです。場所や場面を問わず、おかしいことはおかしいと声を上げることが大切です。また、情報格差による弱い立場の人が苦しまないことを望みます。
同様の問題に直面した場合は、以下の3つのステップをお勧めします。
① 医療ソーシャルワーカー(MSW=メディカル・ソーシャル・ワーカー)に相談する。すべての病院には医療ソーシャルワーカー(MSW)が配置されています。病院を訪れ、受付などで「医療ソーシャルワーカーに相談したい」と伝えてください。ご自身の懸念や疑問をMSWに説明しましょう。多くの場合、MSWが問題解決を支援してくれます。説明に納得できない場合は、請求額をすぐに支払わないようにしてください。
② 医療安全支援センターに相談する。各都道府県には「医療安全支援センター」が設置されています。そこへ苦情や相談を申し立ててください。医療機関と患者の間に立ち、問題解決を支援する非常に頼りになる機関です。
③ 消費生活センターに相談する。②と並行して進めることができます。お住まいの地域の「消費生活センター」を探し、事前見積書と最終請求書を持参して相談してください。消費生活センターは非常に強力な相談機関です。事情を詳しく確認したうえで、病院に対して請求内容の適切な説明を求めたり、請求額についての交渉を支援したりしてくれます。必要に応じて、法的手続きについての助言や支援を受けられる場合もあります。
また、ご加入の保険会社に相談することで、診療報酬点数の計算や請求内容が適切であるかどうかを確認してもらうこともできます。
